Intake Process
We know that when you contact us that it is a difficult time in your life or the life of
one of your loved ones. To make the process faster and easier, please have available all medical and chemical dependency treatment history, demographic information, and ALL active insurance policy(ies) information available.
A referral from a screening agency or your personal medical doctor(s) is very helpful. A referral from your Primary Care Physician (PCP) will likely be required by your insurance provider if you are covered by an HMO plan.
Please know that most insurance carriers take up to 2-3 business days to process a pre-authorization request. Once approved, you will be contacted to schedule the start of your journey to recovery.

What to bring with you
Medication
Detailed list of all medications you are currently taking. Include the name of medication, the dosage and frequency administered, name of physician who prescribed the medication, and the reason for the medication. Only bring a few days’ supply of your prescribed medications when traveling to the facility for admission. All medications must be in the original pharmacy bottle with the information label intact. All liquid medications must be new and sealed.Clothing
Dress for comfort. Bring at least 5-7 different changes of comfortable weather-appropriate clothing and 2-3 pair of pajamas. This will depend on the level of admission and length of stay. Laundry facilities are available for use by patients at no charge. Flat-soled or tennis shoes are recommended and flip flops for the shower.Toiletries
Hygiene items such as any special soaps, shampoos, hair conditioners, face soaps, etc. Electric razor/shaver is the only type allowed. (Note: All electrical items, such as hair dryers, curling irons, or electric shavers will be checked for safety.)Miscellaneous
Appropriate reading materials (books, magazines, a bible, etc.) We ask that you leave all items that are considered valuable at home as the facility will not be responsible for lost or stolen items. If you smoke, please bring enough for the duration of your stay. If you need a specific laundry detergent, please bring that.What NOT to bring with you
It is important to know that upon admission, items will be checked to secure the well-being of yourself and all other patients. Items not on the permissible list above will be taken and the facility will provide storage for personal possessions to be locked away. (Example (jewelry, banking cards, etc.) until the patient is discharged.
Items specifically excluded:
- Products containing alcohol (i.e., mouthwash, aftershave, facial toner, perfume, perfumed lotions, etc.)
- Food, drinks, chewing gum, breath mints, candy of any kind, sugar, or artificial sweeteners.
- Insulated coffee
- Smokeless tobacco, cigars, rolling tobacco, pipes or vapes.
- Illegal substances or mood-altering drugs
- Firearms and/or supplies
- Stuffed animals
- Shoe polish, liquid soap, detergent, or bleach
- Glass items
- Disposable razors, straight razors, razorblades, pocketknives,
- Knitting needles, crochet hooks, or metal hairpieces
- Prescription and non-prescription medications, vitamins, herbs, or nutritional supplements. (Only bring a few days’ supply of your prescribed medications when traveling to the facility for )
- Excessive or expensive jewelry
- Electronics such as tablets, phones, computers, watches, etc.
- Clothing that does not obey the dress code.
