To ensure a faster and easier admission process, having certain information available when you first contact us will be very helpful. This allows our admissions team to quickly verify your needs and insurance benefits.
Please have the following information readily available:
Medical History: Detailed history of all past and current medical conditions.
Chemical Dependency Treatment History: A comprehensive overview of any previous substance abuse treatment, including dates, facilities, and outcomes.
Demographic Information: Your full legal name, date of birth, current address, contact phone number, and emergency contact details.
Active Insurance Policy(ies) Information:
Insurance company name
Policy number
Group number
Member ID
Subscriber’s name and date of birth (if different from the patient)
Customer service phone number on the back of the card
Referral Information (Helpful, Potentially Required):
A referral from a screening agency or your personal medical doctor(s) is very helpful in expediting the process. Please note that a referral from your Primary Care Physician (PCP) will likely be required by your insurance provider if you are covered by an HMO plan.
Understanding Insurance Pre-Authorization:
Please be aware that most insurance carriers take approximately 2-3 business days to process a pre-authorization request for addiction treatment services. Once your pre-authorization is approved, you will be contacted immediately to schedule the start of your journey to recovery at Revive Recovery Center of Beaumont.
To ensure your comfort and focus on recovery, please pack thoughtfully. The amount and type of clothing may depend on your specific level of admission (e.g., Inpatient Medical Managed Detox, Inpatient Residential Treatment) and the length of your stay.
Medication:
A detailed list of all medications you are currently taking. Include the name of medication, the dosage and frequency administered, the name of the physician who prescribed the medication, and the reason for the medication.
Only bring a few days’ supply of your prescribed medications when traveling to the facility for admission. All medications must be in the original pharmacy bottle with the information label intact. All liquid medications must be new and sealed. Our medical staff will manage your medications during your stay.
Clothing:
Dress for comfort. Bring at least 5-7 different changes of comfortable, weather-appropriate clothing.
Include 2-3 pairs of pajamas.
Laundry facilities are available for use by patients at no charge.
Flat-soled or tennis shoes are recommended, and flip-flops for the shower.
Toiletries:
Hygiene items such as any special soaps, shampoos, hair conditioners, face soaps, etc.
An electric razor/shaver is the only type allowed. (Note: All electrical items, such as hair dryers, curling irons, or electric shavers will be checked for safety upon arrival.)
Important: Ensure all toiletries are alcohol-free.
Miscellaneous:
Appropriate reading materials (books, magazines, a bible, etc.).
If you smoke, please bring enough cigarettes for the duration of your stay.
If you need a specific laundry detergent, please bring that.
It is important to know that upon admission, all personal items will be checked to secure the well-being and safety of yourself and all other patients at our facility. Items not on the permissible list above will be taken and the facility will provide secure storage for personal possessions (e.g., jewelry, banking cards, excessive cash) to be locked away until the patient is discharged.
Items specifically excluded and prohibited include:
Products Containing Alcohol: Mouthwash, aftershave, facial toner, perfume, perfumed lotions, hand sanitizers, or any other products with alcohol as an ingredient.
Food & Beverages: Food, drinks, chewing gum, breath mints, candy of any kind, sugar, or artificial sweeteners.
Insulated Coffee/Drink Containers.
Tobacco Products (Other than cigarettes): Smokeless tobacco, cigars, rolling tobacco, pipes, vapes, or e-cigarettes.
Illegal Substances or Mood-Altering Drugs: Any illicit drugs, non-prescribed medications, or substances intended to alter mood.
Weapons: Firearms, knives, or any other supplies that could be considered weapons.
Stuffed Animals.
Cleaning Supplies: Shoe polish, liquid soap (other than personal hygiene), detergent (unless specific for personal laundry), or bleach.
Glass Items: Any items made of glass (e.g., glass bottles, mirrors).
Sharp Objects: Disposable razors, straight razors, razorblades, pocketknives, knitting needles, crochet hooks, or metal hairpieces.
Unauthorized Medications: Prescription and non-prescription medications, vitamins, herbs, or nutritional supplements not approved by our medical staff upon admission. (Only bring a few days’ supply of your prescribed medications when traveling to the facility for admission; all others will be managed by us).
Excessive or Expensive Jewelry.
Electronics: Tablets, personal phones, computers, smartwatches, or any other electronic devices.
Inappropriate Clothing: Clothing that does not obey the facility’s dress code (e.g., overly revealing, offensive imagery).
